It begins from organizations that carry a Glassdoor rating, then attaches their hiring, which surfaces national operators that recruit privately and so fall outside the postings feed. The hiring count is home-health openings from the live feed where the operator appears there, and open roles listed under the corporate name on Glassdoor otherwise.
